Shift/Hours Full-time, 9 a.m. to 5 p.m. Monday to Friday

Job Summary
As a member of the interdisciplinary care management team, the Discharge Planner RN is accountable and responsible for coordinating the discharge of all short-term stay residents, promoting effective utilization and monitoring of health care resources, and working with the interdisciplinary team to achieve desired clinical, financial, and resource outcomes. Will assist, as needed, in the completion of nursing admission assessments and Minimum Data Set (MDS). Demonstrates an understanding of, and embraces, the mission statement of Christian Health Care Center.

Education A graduate of an accredited RN school of nursing or a nursing degree from an accredited college or university. Bachelor of Science in Nursing preferred.

Previous Experience
Skilled in assessing, planning, and managing resident after care as acquired through three years of clinical experience in long-term care/acute care environment. MDS, utilization management/quality management experience preferred. Interpersonal skills to obtain and interpret information appropriate to residents’ needs and age as required for assessment, range of treatment, and resident care a must.

Special Skills/Licenses
Must possess a current, unencumbered license as a Registered Nurse in the State of New Jersey or authorized by permit to practice as a Registered Nurse in the State of New Jersey.
